Referral Pathway Mapping
When a patient raises a concern about intimacy, sexual health, body image, relationships, pelvic health, psychosocial wellbeing, or a related need, does your team know where to refer them?
Referral Pathway Mapping is a two-hour structured organizational session that helps cancer programs answer that question more clearly.
CancerEVOLVE guides your team through the process of identifying the support already available, clarifying who does what, understanding how referrals currently happen, and identifying important gaps or points of confusion.
What we explore
Together, we map resources across the broader support ecosystem, including:
- People and services within your immediate team or program
- Resources elsewhere within your organization or health system
- Local and regional providers and community organizations
- National, virtual, and educational resources
We also explore which professionals and services are appropriate for different concerns, how patients currently reach them, and where your team may need additional options or greater clarity.
The purpose is not simply to build a dynamic directory. It is to create a more useful understanding of who can help with what and how patients can access that support.

- A visual overview of your referral ecosystem
- An editable living referral matrix organizing internal and external resources
- Greater clarity around roles and referral options
- Identification of important gaps and points of confusion
- A concise summary of key findings
- Three to five prioritized next steps for the following 30 to 90 days
- A 30-minute follow up and review of the completed materials and recommendations
How it works
Before the session, your organization completes a structured intake and shares relevant existing referral and resource information.
During the two-hour facilitated session, CancerEVOLVE guides a small group of team members through the mapping process.
Afterward, the information is organized into your referral ecosystem materials, findings, and recommended next steps.

The standard virtual engagement includes preparation, review of existing materials, the two-hour facilitated mapping session, completed referral ecosystem materials and recommendations, and a 30-minute follow up.
On-site facilitation, larger multi-department and multi-location projects are available through a customized proposal.
